INSURANCE INFORMATION
Proof of Insurance
We ask that you present your insurance card to us at every visit. If you fail to provide us with the correct insurance information at each visit, you may be responsible for payment for all services provided.
If your insurance company requires you to pick a Primary Care Physician (PCP), one of our physicians must be the PCP listed on your insurance card. The office staff can provide you with the appropriate physician identification numbers for your insurance.
We are contracted with most insurance plans. View the list of plans which we participate.
Your health insurance contract is between you and your insurance company. Knowing your insurance benefits is your responsibility. Any questions or complaints regarding your coverage should be directed to your insurance carrier. You are also responsible for knowing which diagnostic facilities for laboratory, radiology and hospitalization are covered by your insurance.
Co-Payments
Your insurance company requires us to collect co-payments at the time of service.
Claims Submission
We will submit your claims and assist you in any way we reasonably can to help get your claims paid. Your insurance company may need you to supply certain information directly. It is your responsibility to comply with their request.

Most major health insurance carriers require participants to obtain a referral authorization from his/her primary care physician (PCP) before seeking care in a specialist's office. You may be required to see your physician before your insurance will authorize it.
It is the patient's responsibility to notify Brunswick Internal Medicine of a referral request prior to seeing a specialist for medical care. If your plan is an HMO product, please call the office to request a referral. If you are unsure whether your plan requires referrals, please check with your insurance company directly.
Most referrals can and will be processed within 72 hours. Same day referral requests will only be processed if authorized by the physician as an emergency.
